Message from the Athletic Director
George Read Middle School Sports for the 2023-24 School Year
All students must have a valid DIAA physical dated 4/1/23 or later to participate in any sport at George Read.
In order to be considered for a position on any sports team, each student must have the Colonial School District Sports Permission Form and the DIAA Physical Form (dated after 4/1/23) on file with the Athletic Director, Mr. Sawdey. All pages must be signed & dated where indicated. The DIAA forms may be found by using the link on this page, or they may be obtained in the Main Office at school. Colonial School District Permission Forms are also available in the Main Office.
* The DIAA physical is valid for one year from the time of the physical examination.
Students may have their Family Doctor, Walgreen’s, or Medexpress perform the physicals, but they must be documented on the DIAA Physical Form.

Physicals at George Read
George Read will offer physical exams through the Life Health Center on Wednesday, September 6, 2023 for a fee of $25 cash. Parents and students must complete Pages 2 & 3 of the Life Health packet and The DIAA Physical Form pages 1 & 2, and sections 1-3 on page 3, and page 5. These forms must be presented to the physician at the time of the physical. The Life Health packet and DIAA forms are available from your child’s homeroom teacher, or from the Main Office.
*Parents/Guardians and students must also complete and return the Individual Sports packet for each sport to be played during the school year. These will be handed out by each coach during that Sport's Interest meeting.
As always, students must maintain a 1.75 GPA in order to be eligible to play a sport at George Read Middle School.
